The American Academy of Pediatrics states: "Care coordination is a process that links children with special health care needs and their families to services and resources in a coordinated effort to maximize the potential of the children and provide them with optimal health care".
We believe that a parent/caregiver is the only constant person throughout a child's life transitions. From hospital to home, to aging, to education, to work, the parent/caregiver knows first hand the needs of the child. Our center exists to support families of CSHCN throughout the child's life transitions (hospital to home, age and school changes).
Care Coordination Services are available via our website, phone consultations, and meetings (at home or hospital). Our Director of Care Coordination will be able to assist families and health care professionals with planning and implementing services, managing data and medical records, care plan support and in teaching overall care-giving skills.
