Transitional Care is the bridge between hospital and home for a child who is clinically ready for discharge from a hospital, yet important factors need to be considered in order to bring the child home safely.
Respite Care is a short term stay (24 hours to 2 weeks) for a child who has medical complexities and/or may be technology assisted.
