Transitional Care

Transitional care is short term stay of up to 120 days that helps bridge the transition form hospital to home. It is designed for children who are medically ready to leave the hospital but whose families may need additional time, training , or resources to ensure a safe return home. During this period, families receive support with caregiver training and education, medical equipment training, and coordination of hoe nursing services to help prepare for their child’s ongoing care needs.
